DV Document Log

Refer to Section 6 of the OAI (Appendix E) on how to report each component and Appendix A of the OAI for a document log template and examples.

This section of the OAI (Appendix E) has the document log DVCs use as an inventory of all documents and files that the SO delivers.

The document log has the following 5 components for the SO to list information about each document:

  • Reporting section
  • Document file name
  • Document type
  • Reporting stage
  • Document description

Now that we’ve learned how to complete the OAI, let’s learn about preparation activities for the on-site or virtual visit.
