EES Example
This is an example of the Part C Grievances form of the EES. DVCs should capture review results for each data element when weights are connected with standard/sub-standard descriptions.

This is a screenshot of the Part C Grievances form of the EES. Column 1 lists the Standard/Sub-standard ID, which is 1.a. Column 2 lists the Standard/Sub-standard Description which says that SOs properly secured source documents and output, and have referenced data file locations correctly so that DVCs can retrieve source documents at any time to validate the information submitted to CMS via CMS systems. Column 3 is the Data Elements column which is empty. Column 4 is named Data Sources and Review Results: Enter review results and/or data sources. Review Results: is listed in that column. Column 5 is the final column and is named Enter ‘Findings’ using the applicable choice in the appropriate cells. Cells marked with an asterisk should not be edited. This column is empty.
