2025 Medicare Part C & Part D Reporting Requirements & Data Validation

Submitting Questions to CMS

Email submissions should include the following information, in list format and in the following order:

  • Name of SO
  • CMS contract numbers
  • SO’s contact name, title, phone number, and email address
  • Name of DVC organization

For each area of appeal, list the following information:

  • Part C or Part D, name of reporting section
  • Standard/sub-standard ID
  • Description of DVC’s finding
  • Reason for disagreement with finding
  • Steps that were taken to resolve the disagreement with the DVC before the submission of the findings
  • Outcome of discussions, areas of impasse, and any more information
